Short answer: Retatrutide is an investigational peptide designed to activate GIP, GLP-1 and glucagon receptors. Tirzepatide targets GIP and GLP-1 receptors. The extra target is a mechanistic difference, not proof that one compound is automatically better or safer.
What triple agonism means
An agonist activates a receptor. “Triple receptor agonist” describes a molecule that acts at three receptor types. It does not mean three separate medicines have been mixed together, and it does not mean each receptor is activated equally. Researchers must characterize potency, exposure, receptor selectivity and downstream signaling.
The evidence now extends beyond the 2023 phase 2 trial. Two peer-reviewed phase 3 reports were published online on September 29, 2026:
- TRIUMPH-1 randomized 2,339 adults with obesity without diabetes and assessed outcomes over 80 weeks. Retatrutide produced greater average weight reduction than placebo; gastrointestinal events were the most common adverse events. TRIUMPH-1 original report
- TRIUMPH-2 randomized 1,152 adults with obesity and type 2 diabetes. At 80 weeks, the studied retatrutide groups had greater average weight reduction than placebo. Gastrointestinal events, hypotension and dysesthesia were more frequent with retatrutide. The paper appeared in the October 10 issue of The Lancet. TRIUMPH-2 original report
Both trials were funded by Eli Lilly. They substantially extend the clinical evidence, but neither is a direct comparison with tirzepatide, and publication does not itself confer drug approval.

Adding glucagon-receptor activity changes the scientific hypothesis. It also adds questions about the balance of effects across tissues. A more complicated mechanism can bring different benefits, risks or tradeoffs. The answer has to come from trials, not from counting receptors.
Comparisons across unrelated studies are especially fragile. Two trials may differ in participant health, duration, background care, treatment discontinuation, statistical methods and which results are counted after treatment stops. Head-to-head randomized research is more informative than placing the largest percentage from each trial side by side.
Investigational is not approved
As checked November 4, 2026, FDA states that retatrutide is not a component of an FDA-approved drug and has not been found safe and effective for a condition. FDA also states that retatrutide cannot be used in compounding under federal law. This status must be rechecked before publication because regulatory information can change. A clinical-trial result applies to the tested material and protocol. It does not establish the identity, quality or safety of a product with the same name sold elsewhere. Articles about investigational compounds should therefore explain the research without giving a route to self-treatment.
