Retatrutide liver research has examined changes in liver fat in a randomized phase 2a study. This is human evidence, but the endpoint must be described accurately: an improvement in an imaging measure is not the same as proving that cirrhosis, fibrosis or long-term liver risk has been reversed. Retatrutide phase 2a trial in metabolic liver disease

Keep fat and scarring distinct

Liver fat can be measured repeatedly with imaging, making it useful for studying biological response. Fibrosis concerns scar tissue. Inflammation and cell injury add further dimensions. A person may improve on one measure while retaining important disease risk on another.

A study can therefore provide a strong signal that a candidate affects liver fat while still leaving questions about progression, clinical events and suitability for people with advanced disease. Those questions need their own populations, endpoints and follow-up.

Read the substudy in context

When liver measurements come from a subgroup of a broader trial, examine how participants were selected and whether the subgroup resembles the patients in whom a treatment claim is being made. Also consider whether the analysis was prespecified and whether missing measurements could influence the result.

The treatment landscape is not empty. FDA granted Wegovy injection accelerated approval for adults with noncirrhotic MASH and moderate-to-advanced fibrosis consistent with stages F2–F3. The indication is based on improvement in MASH and fibrosis; confirmatory verification of clinical benefit remains pending, and continued approval may depend on it. This is a product- and population-specific indication, not approval of retatrutide, Wegovy tablets for MASH, or every semaglutide preparation. FDA Wegovy prescribing information January 2026

AXON’s interpretation is that retatrutide’s imaging findings justify continued investigation. They do not establish a general fatty-liver cure, an approved liver indication or a reason to use research material outside clinical oversight. A responsible summary gives the endpoint its correct name and states which clinically important questions remain unanswered.
